How to Complete the Annual Financial Conflict of Interest (FCOI) Disclosure Form in Workday

Completing the FCOI Questionnaire

Step 1: On the Workday home screen, navigate to Awaiting Your Action and select Edit Worker Additional Data Event. This item will also be available via your My Tasks icon. For more information on My Tasks, please visit: Inbox Reimagined in Workday.

Step 2: Read and review the FCOI Disclosure Form instructions and complete the associated questionnaire.

Step 3: To attach any relevant documentation, you can either Drop Files in the file upload section or Select Files on your device.

You can select Upload if you would like to upload additional documents.

Step 4: In the Investigator Certification section, please read and certify by typing your name into the Text Box.

Step 5: Select Submit.
